Einsatz der elektronischen Patientenakte im Operationssaal am Beispiel der HNO-Chirurgie
Wenn ein Chirurg heutzutage wĂ€hrend der Operation Informationen aus der Patientenakte benötigt, ist er gezwungen, sich entweder unsteril zu machen oder Personal anzuweisen, ihm die entspre-chenden Informationen zugĂ€nglich zu machen. Aus technischer Sicht ist ein System zur intraoperati-ven Bedienung und Darstellung sehr einfach zu realisieren. Grundlage dafĂŒr ist eine elektronische Patientenakte (EPA), welche beispielsweise softwaregenerierten oder eingescannten Dokumenten verwaltet. Die vorliegende Arbeit widmet sich den folgenden Fragen: Wird ein solches System im Operationssaal sinnvoll genutzt? Welche Methoden zur sterilen Bedienung kommen infrage? Wie muss die grafische Darstellung auf den Operationssaal abgestimmt werden? Kann durch das Imple-mentieren aktueller Kommunikationsstandards auf alle verfĂŒgbaren Patientendaten zugegriffen werden?
Dazu wurden in einer ambulanten HNO-Klinik zwei Pilotstudien durchgefĂŒhrt. In der ersten Studie wurde das erste auf dem Markt befindliche kommerzielle Produkt âMI-Reportâ der Firma Karl Storz evaluiert, welches per Gestenerkennung bedient wird. FĂŒr die zweite Studie wurde ein EPA-System entwickelt (Doc-O-R), welches eine Vorauswahl der angezeigten Dokumente in AbhĂ€ngigkeit des Eingriffs traf und mit einem FuĂschalter bedient werden konnte. Pro System wurden ca. 50 Eingriffe dokumentiert. Dabei wurde jedes angesehene Dokument und der Nutzungsgrund protokolliert. Die Systeme wurden durchschnittlich mehr als einmal pro Eingriff genutzt. Die automatische Vorauswahl der Dokumente zur Reduzierung der Interaktionen zeigte sehr gute Ergebnisse.
Da das behandelte Thema noch in den AnfĂ€ngen steckt, wird in der Arbeit am Ende auf die Vielzahl von Möglichkeiten eingegangen, welche bezĂŒglich neuartiger Darstellungsmethoden, Bedienvorrich-tungen und aktueller StandardisierungsaktivitĂ€ten noch realisiert werden können. Potentiale, Wirkungen und ökonomische Bewertung von hochwertigen Verkehrsinformationsdiensten
This paper analyzes the high-end range of traffic information systems for individual motorized traffic and for public transport. High value-added traffic services are characterized by dynamic, personalized information, including suggestions for alternative routing; they are supplied on a commercial basis, mainly by private enterprises. Based on a qualitative forecast of the diffusion of high value-added traffic information systems, we estimate their potential impact on the transport system as a whole, as well as on the environment. The approach also considers the strategies of the key actors (mainly enterprises) at each level of the value-added chain: Primary data collection and treatment, distribution of information services, data transfer, and the reception of information via specialized devices. The study concludes that high value-added traffic information systems will be much more widely diffused in the future, both in public transport and in individual motorized traffic. Traffic information services will also lead to a more efficient use of infrastructure. However, as regards economic policy, there seems to be no need for state intervention to remedy market failure. -- In diesem Beitrag werden hochwertige Verkehrsinformationsdienste fĂŒr Nutzer des motorisierten Individualverkehrs (MIV) und des ĂPNV untersucht, die aus kommerziellen Interessen bereitgestellt werden und sich durch dynamische, personalisierte und empfehlende Verkehrsinformationen auszeichnen. Aufbauend auf einer qualitativen Prognose ĂŒber die zukĂŒnftige Verbreitung hochwertiger Verkehrsinformationsdienste werden deren Auswirkungen auf das System Verkehr und die Umwelt ermittelt, um dann einen eventuellen wirtschaftspolitischen Handlungsbedarf abzuleiten. Dabei werden auch die Strategien der SchlĂŒsselakteure auf den MĂ€rkten in die Ăberlegungen miteinbezogen. Bei der Analyse werden explizit die einzelnen Stufen der Wertschöpfungskette von Verkehrsinformationsdiensten betrachtet: Verkehrsdatenerhebung und -aufbereitung, Angebot und Vertrieb der Verkehrsinformationsdienste, DatenĂŒbertragung per Mobilfunk sowie Informationsausgabe auf Telematik-EndgerĂ€ten. Es wird festgestellt, dass zukĂŒnftig hochwertige Verkehrsinformationsdienste sowohl im ĂPNV wie auch im MIV eine weite Verbreitung erreichen und tendenziell zu einer effizienteren Nutzung der Infrastruktur beitragen werden. Focal Seizures and Posterior Reversible Encephalopathy Syndrome as Presenting Signs of IgA Vasculitis/Henoch-Schoenlein PurpuraâAn Educative Case and Systematic Review of the Literature
Background: IgA vasculitis/Henoch-Schoenlein purpura (IgAV/HSP) is a systemic small
vessel vasculitis of unknown pathogenesis predominantly affecting children. While skin,
GI tract, joints, and kidneys are frequently affected and considered, central nervous
system (CNS) involvement of this disease is underestimated.
Methods: We provide a case report and systematically review the literature on IgAV,
collecting data on the spectrum of neurological manifestations.
Results: We report on a 7-year-old girl with IgAV who presented with diplopia
and afebrile focal seizures, which preceded the onset of purpura. Cranial magnetic
resonance imaging was consistent with posterior reversible encephalopathy syndrome
(PRES), showing typical focal bilateral parietal swelling and cortical and subcortical high
signal intensities on T2-fluid attenuated inversion recovery (FLAIR) images predominantly
without diffusion restriction. Cerebrospinal fluid analysis and blood tests excluded
systemic inflammation or vasculitis. Interestingly, hypertension was not a hallmark of
the developing disease in the initial phase of PRES manifestation. Renal disease and
other secondary causes for PRES were also excluded. Supportive- and steroid treatment
resulted in restitution ad integrum. Reviewing the literature, we identified 28 other cases of
IgAV with CNS involvement. Severe CNS involvement includes seizures, cerebral edema,
or hemorrhage, as well as PRES. Thirteen patients fulfilled all diagnostic criteria of PRES.
The mean age was 11.2 years (median 8.0, range 5-42 years), with no reported bias
toward gender or ethnic background. Treatment regimens varied from watchful waiting
to oral and intravenously steroids up to plasmapheresis. Three cases showed permanent
CNS impairment.
Conclusion: Collectively, our data demonstrate that (I) severe CNS involvement such
as PRES is an underappreciated feature of IgAV, (II) CNS symptoms may precede other
features of IgAV, (III) PRES can occur in IgAV, and differentiation from CNS vasculitis is
challenging, (IV) pathogenesis of PRES in the context of IgAV remains elusive, which
hampers treatment decisions. We, therefore, conclude that clinical awareness and
the collection of structured data are necessary to elucidate the pathophysiological
connection of IgAV and PRES
Elektronenbehandlung im Getreide - PhytosanitĂ€re Wirkung der Elektronenbehandlung im Getreide im Zusammenhang mit nachfolgenden FungizidmaĂnahmen sowie Untersuchungen zur Eignung des Verfahrens zur ErfĂŒllung der Beizbedingung ; Abschlussbericht zum Forschungsvorhaben
Die Elektronenbehandlung von Getreidesaatgut wird seit einigen Jahren als eine Alternative zur chemischen Beizung angeboten. Diese physikalische Saatgutbehandlung beruht auf der bioziden Wirkung niederenergetischer Elektronen und stellt eine Möglichkeit dar, im Sinne des integrierten Pflanzenschutzes den Einsatz von chemischen Wirkstoffen auf das notwendige MaĂ zu beschrĂ€nken. Die Technologie der Elektronenbehandlung ist unter dem Namen e-eventusÂź bekannt und wurde insbesondere in Sachsen entwickelt und gefördert. Im Rahmen eines zweijĂ€hrigen Forschungsprojektes wurde die EinfĂŒhrung dieses alternativen Pflanzenschutzverfahrens in Sachsen in die Praxis begleitet. Zielsetzung war eine RisikoabschĂ€tzung bezĂŒglich einer Befallsausbreitung und Anreicherung samen- und bodenbĂŒrtiger Pathogene, die durch die Elektronenbehandlung nicht oder nur unzureichend erfasst werden. Weiterer Projektinhalt war eine ökologische und ökonomische Bewertung des Verfahrens sowie die Ableitung von Anwendungsempfehlungen fĂŒr den Landwirt. AuĂerdem wurde die EffektivitĂ€t der Elektronenbehandlung im Vergleich zu herkömmlichen chemischen Beizen geprĂŒft und dies im Zusammenhang mit nachfolgenden FungizidmaĂnahmen. Ein weiteres Projektziel, dass im Verfahren der Saatenanerkennung neben der chemischen Beizung auch die Elektronenbehandlung unter bestimmten Vorraussetzungen als Saatgutbehandlungsverfahren bundesweit anerkannt ist, wurde bereits im Juni 2005 umgesetzt. Die im Projekt gewonnenen Ergebnisse zeigen, dass die sachgerechte, situationsbezogene Anwendung der Elektronenbehandlung zu keinerlei Nachteilen bei den Ertrags- und QualitĂ€tsparametern bei der Produktion von Winterweizen und -gerste im Vergleich zur chemischen Beizung fĂŒhrt. Die nachfolgenden Fungizidstrategien waren unabhĂ€ngig vom Verfahren der Saatgutbehandlung. Die Elektronenbehandlung wird als umweltfreundliches Verfahren zur Saatgutbehandlung von Z-Saatgut in Sachsen empfohlen. Dies gilt unter Beachtung der bekannten WirkungslĂŒcken bzw. -schwĂ€chen, wie die FlugbrĂ€nde und bodenbĂŒrtige Pathogene. Der Einsatz von e-behandeltem Saatgut in den Vorgebirgs- und Mittelgebirgsregionen in Sachsen wird auf Grund der erhöhten Befallsgefahr durch Microdochium nivale nicht empfohlen. Eine kontinuierliche Beobachtung der BestĂ€nde auf samen- und bodenbĂŒrtige Erreger ist in Betrieben mit einem hohen Anteil an e-behandeltem Saatgut erforderlich. AuĂerdem wird ein Wechsel zwischen den verschiedenen Saatgutbehandlungsverfahren im Betrieb als sinnvoll erachtet. Bei eigenem Nachbau sollte stets eine SaatgutqualitĂ€tsbestimmung mit anschlieĂender Saatgutbehandlung durchgefĂŒhrt werden. Die Saatgutbehandlung ist und bleibt als wichtige VorsorgemaĂnahme ein unverzichtbarer Bestandteil der integrierten Pflanzenproduktion
Cacao flower visitation: Low pollen deposition, low fruit set and dominance of herbivores
Pollination services of cacao are crucial for global chocolate production, yet remain critically understudied, particularly in regions of origin of the species. Notably, uncertainties remain concerning the identity of cacao pollinators, the influence of landscape (forest distance) and management (shade cover) on flower visitation and the role of pollen deposition in limiting fruit set. Here, we aimed to improve understanding of cacao pollination by studying limiting factors of fruit set in Peru, part of the centre of origin of cacao. Flower visitors were sampled with sticky insect glue in 20 cacao agroforests in two biogeographically distinct regions of Peru, across gradients of shade cover and forest distance. Further, we assessed pollen quantities and compared fruit set between naturally and manually pollinated flowers. The most abundant flower visitors were aphids, ants and thrips in the north and thrips, midges and parasitoid wasps in the south of Peru. We present some evidence of increasing visitation rates from medium to high shade (40%â95% canopy closure) in the dry north, and opposite patterns in the semi-humid south, during the wet season. Natural pollination resulted in remarkably low fruit set rates (2%), and very low pollen deposition. After hand pollination, fruit set more than tripled (7%), but was still low. The diversity and high relative abundances of herbivore flower visitors limit our ability to draw conclusions on the functional role of different flower visitors. The remarkably low fruit set of naturally and even hand pollinated flowers indicates that other unaddressed factors limit cacao fruit production. Such factors could be, amongst others, a lack of effective pollinators, genetic incompatibility or resource limitation. Revealing efficient pollinator species and other causes of low fruit set rates is therefore key to establish location-specific management strategies and develop high yielding native cacao agroforestry systems in regions of origin of cacao. © 2022 The Authors. The STAGES view of red spirals and dusty red galaxies: Mass-dependent quenching of star-formation in cluster infall
We investigate the properties of optically passive spirals and dusty red
galaxies in the A901/2 cluster complex at redshift ~0.17 using restframe
near-UV-optical SEDs, 24 micron IR data and HST morphologies from the STAGES
dataset. The cluster sample is based on COMBO-17 redshifts with an rms
precision of sigma_cz~2000 km/sec. We find that 'dusty red galaxies' and
'optically passive spirals' in A901/2 are largely the same phenomenon, and that
they form stars at a substantial rate, which is only 4x lower than that in blue
spirals at fixed mass. This star formation is more obscured than in blue
galaxies and its optical signatures are weak. They appear predominantly in the
stellar mass range of log M*/Msol=[10,11] where they constitute over half of
the star-forming galaxies in the cluster; they are thus a vital ingredient for
understanding the overall picture of star formation quenching in clusters. We
find that the mean specific SFR of star-forming galaxies in the cluster is
clearly lower than in the field, in contrast to the specific SFR properties of
blue galaxies alone, which appear similar in cluster and field. Such a rich red
spiral population is best explained if quenching is a slow process and
morphological transformation is delayed even more. At log M*/Msol<10, such
galaxies are rare, suggesting that their quenching is fast and accompanied by
morphological change. We note, that edge-on spirals play a minor role; despite
being dust-reddened they form only a small fraction of spirals independent of

STAGES: the Space Telescope A901/2 Galaxy Evolution Survey
We present an overview of the Space Telescope A901/2 Galaxy Evolution Survey
(STAGES). STAGES is a multiwavelength project designed to probe physical
drivers of galaxy evolution across a wide range of environments and luminosity.
A complex multi-cluster system at z~0.165 has been the subject of an 80-orbit
F606W HST/ACS mosaic covering the full 0.5x0.5 (~5x5 Mpc^2) span of the
supercluster. Extensive multiwavelength observations with XMM-Newton, GALEX,
Spitzer, 2dF, GMRT, and the 17-band COMBO-17 photometric redshift survey
complement the HST imaging. Our survey goals include simultaneously linking
galaxy morphology with other observables such as age, star-formation rate,
nuclear activity, and stellar mass. In addition, with the multiwavelength
dataset and new high resolution mass maps from gravitational lensing, we are
able to disentangle the large-scale structure of the system. By examining all
aspects of environment we will be able to evaluate the relative importance of
the dark matter halos, the local galaxy density, and the hot X-ray gas in
driving galaxy transformation. This paper describes the HST imaging, data
reduction, and creation of a master catalogue. We perform Sersic fitting on the
HST images and conduct associated simulations to quantify completeness. In
addition, we present the COMBO-17 photometric redshift catalogue and estimates
of stellar masses and star-formation rates for this field. We define galaxy and
cluster sample selection criteria which will be the basis for forthcoming
science analyses, and present a compilation of notable objects in the field.
Finally, we describe the further multiwavelength observations and announce

Interacting Galaxies in the A901/902 Supercluster with STAGES
We present a study of galaxy mergers and the influence of environment in the
Abell 901/902 supercluster at z~0.165. We use HST ACS F606W data from the
STAGES survey, COMBO-17, Spitzer 24um, and XMM-Newton X-ray data. Our analysis
utilizes both a visual classification system, and quantitative CAS parameters
to identify systems which show evidence of a recent or ongoing merger of mass
ratio >1/10. Our results are: (1) After visual classification and minimizing
the contamination from false projection pairs, we find that the merger fraction
f_merge is 0.023+/-0.007. The estimated fractions of likely major mergers,
likely minor mergers, and ambiguous cases are 0.01+/-0.004, 0.006+/-0.003, and
0.007+/-0.003, respectively. (2) The mergers lie outside the cluster core of
radius R < 0.25 Mpc: the lack of mergers in the core is likely due to the large
galaxy velocity dispersion in the core. Mergers populate the region (0.25 Mpc <
R <= 2 Mpc) between the core and outskirt. In this region, the estimated
frequency of mergers is similar to those seen at typical group overdensities.
This suggests ongoing growth of the clusters via accretion of group and field
galaxies. (3) We compare our observed merger fraction with those reported in
other clusters and groups out to z~0.4. Existing data points on the merger
fraction for L<= L* galaxies in clusters allow for a range of evolutionary
scenarios. (4) The fraction of mergers, which lie on the blue cloud is
80%+/-18% versus 34%+/-7% for non-interacting galaxies, implying that
interacting galaxies are preferentially blue. (5) The average SFR, based on UV
or UV+IR data, is enhanced by a factor of ~1.5 to 2 in mergers compared to
non-interacting galaxies. However, mergers in the clusters contribute only a
